Prior to scheduling any appointments, all mandatory intake forms must be completed in their entirety. Please complete these forms via the provided links above. Dynamic Speech & Language uses FormDr which is a HIPAA compliant platform ensuring all patient data is encrypted in transit and at rest. You can sign and complete these forms with any device, anywhere, with confidence that your personal information is protected.
After all forms are completed, you will receive an email (within 3-4 business days) from Theresa, Dynamic Speech & Language Therapy’s billing/insurance specialist. This email will contain a breakdown of your deductible, coinsurance amounts, preauthorization requirements, copay, number of visits, etc. to ensure you are informed and have a solid understanding of potential costs associated with speech/language services.
