STEP 1: Contact Christy today to schedule a free consultation. Once the consultation has been completed, and it has been determined that Dynamic Speech & Language Therapy is a good fit for your needs, please go to the website’s dropdown menu, titled “forms”, to complete the mandatory intake forms.
STEP 2: Completing the mandatory intake forms must be done prior to scheduling any appointment. Complete your forms via the provided links on the website. Dynamic Speech & Language uses FormDr, which is a HIPAA compliant platform ensuring all patient data is encrypted in transit and at rest. You can sign and complete these forms with any device, anywhere, with confidence that your personal information is protected.
STEP 3: After all forms are completed, you will receive an email (within 3-4 business days) from Theresa, Dynamic Speech & Language Therapy’s billing/insurance specialist. This email will contain a breakdown of your deductible, coinsurance amounts, preauthorization requirements, copay, number of visits, etc. to ensure you are informed and have a solid understanding of potential costs associated with speech/language services. At that time, you will also receive an email containing information on how to access the HIPAA compliant patient portal to set up your auto payments, view upcoming appointments, review invoices, etc..
STEP 4: Once benefits are verified, preauthorization’s are obtained, and all forms are completed in their entirety, then Christy, the Speech-Language Pathologist, will contact you to schedule the initial evaluation.
STEP 5: Once the evaluation is completed, Christy will discuss those findings with the family/client to determine service delivery options (teletherapy or in person) and discuss treatment priorities.
